Steps to registertop ↑

  1. Complete the “Personal Data Form” and submit Personal Data Form
  2. The system will then send you a password by e-mail which is needed to complete the process of registration (conference registration, accommodation and tours)
  3. Click on the ‘Registration Form’ button which will take you to the Registration Fee screen. On completion of this page click ‘next’ which will take you to the ‘Accommodation’ screen and so on until your preferences and payment is ready for submission.
  4. After submitting the registration form you will receive a Letter of Confirmation via e-mail or fax within 3 working days. PLEASE NOTE THE ON LINE PAYMENT IS POSSIBLE ONLY AFTER YOU RECEIVE THE LETTER OF CONFIRMATION.
  5. Present the Letter of Confirmation at the Registration desk at the conference venue to receive your Conference Bag and knowledge pack.

Cancellation policytop ↑

The Conference Organisor must be notified in writing via email about the cancellation of a registration. Refunds where appropriate will only be made after the Conference with the following conditions:

Cancellations received before 15 February 20121 500 CZK cancellation fee
Cancellations received after 15 February 201250% cancellation fee
Cancellations received after 26 May 2011100% cancellation fee

Alternate delegatetop ↑

If a registered and fully paid delegate is unable to attend the conference there is an opportunity for a colleague to assume the role of attendee.. Name changes should be notified in writing to Conference Secretariat. A handling fee of 500 CZK will be charged for each name change.

Registration fees must be paid in Czech Crowns (CZK) by one of the following methods:

1. Credit Cards (MasterCard/Eurocard, Visa, Diners Club and American Express)

Online payment: MasterCard/Eurocard, Visa and Diners Club cards are accepted

Please use the Online Payment System (available after login).

Payment via Authorization Form

To use American Express cards please complete the Credit Card Payment Authorization Form and kindly send it signed by a card holder to the Congress Secretariat by fax (+420 284 001 448) or scanned copy via e-mail (ifa2012registration@guarant.cz).

Payments will be charged in local currency – Czech Crowns (CZK) at the exchange rate of the Czech National Bank on the date of payment.
